Preparing for your appointment:

- Download and complete the Welcome to Our Clinic Form. (Descargar y completar el formulario Bienvenidos a Nuestra Clínica)
- Bring any insurance information and forms you might have.
- Bring your “old” eyeglasses and contact lenses.
- Bring the medicines or a list of the medicines you are taking-including prescription and non-prescription drugs, vitamins, etc.
- Our staff is fluent in Spanish and English. If other language is preferred, please bring your own interpreter.

What to expect on the day of your appointment:
- Please arrive 10-15 minutes before your appointed time to complete the necessary paperwork.
- The length of your examination varies depending on the services you need. Generally a comprehensive examination may take about 90 minutes.
